From 3 August 2026, all medical and admin requests should be submitted using our online form. No internet or smartphone? Call or visit reception and we'll complete the form for you. Every request is assessed fairly in the same way.

What is Accurx?

Accurx is a secure NHS-approved system used by most GP practices in the UK to communicate with patients. It works with our clinical system and lets you send requests to the practice online.

What is Total Triage?

Total Triage means every medical request is sent to our clinical team first. They will review your information and decide the best care for you. This may include:

  • An appointment with the right clinician.
  • Advice without needing an appointment.
  • Referral to another service (such as a pharmacy or physiotherapy).
  • Arranging tests before you are seen.

Why are we changing?

We're introducing Total Triage to:

  • Reduce long telephone queues.
  • Make it easier to contact the practice.
  • Ensure patients with the most urgent needs are seen first.
  • Help you get the right care at the right time.

When can I submit a request?

Medical and admin request forms are available:

  • Monday to Friday
  • 8:00am to 6:30pm

If the practice reaches safe working capacity, the form may close early. The surgery will still be open until 6:30pm for urgent clinical matters.

For emergencies, always call 999 or attend your nearest Emergency Department.

Do I need to download an app or create an account?

No. You can complete the form directly through the Woodside Medical Centre website. No app, username or password is needed.

How long does the form take to complete?

The form is quick and asks about:

  • Your symptoms.
  • How long you've had them.
  • What help you need.
  • How we can contact you.

I don't have internet access. What can I do?

A friend, family member or carer can complete the form for you.

If this isn't possible, our Care Navigator team can help complete the form over the telephone.

How do I access the form?

Can someone complete the form on my behalf?

Yes. The form asks if someone else is completing it for you.

Please note that we can only discuss your care with another person if you have given consent and this is recorded on your medical record.

Can I include more than one health problem on the same form?

Please submit one form for each health concern. This helps the clinical team assess each problem properly.

What happens after I submit my request?

Our team will review your request and decide the best next step. You may:

  • Receive advice by text or phone.
  • Be asked for more information.
  • Be offered an appointment.
  • Receive a booking link by text.
  • Be directed to another suitable NHS service, such as a pharmacy or physiotherapy.

I'm housebound. How do I request a home visit?

You should still submit a triage request online or by telephone with our Care Navigator team.

If you think you need a home visit, please contact the practice before 10:30am where possible.

What if I need a routine appointment?

Routine appointments are requested by completing a medical request form through our website. This helps us make sure you are seen by the right clinician in the most appropriate timeframe.
